Job Description

Role Summary

We are seeking a dedicated and compassionate Occupational Therapist to join our care team and help patients achieve their highest level of independence and quality of life. In this role, you will evaluate, plan, and implement individualized therapy programs, working closely with patients, families, and the care team to support functional improvements and daily living skills. Duties / Responsibilities

  • Assess patients’ physical, cognitive, and emotional abilities to identify challenges and create individualized therapy plans.
  • Develop and implement treatment programs designed to improve patients’ daily living skills, mobility, and independence.
  • Provide hands‑on therapy, education, and training to patients and their families to support functional progress.
  • Collaborate with the care team, physicians, and other healthcare professionals to coordinate comprehensive patient care.
  • Monitor and document patient progress, adjusting treatment plans as needed.
  • Recommend adaptive equipment, assistive devices, and environmental modifications to enhance patient safety and function.
  • Educate patients and families on strategies to maintain and improve functional abilities outside of therapy sessions.
  • Ensure compliance with all regulatory standards, safety protocols, and HIPAA requirements.
  • Participate in continuing education, professional development, and team meetings.
  • Perform other related duties as assigned.

Required Qualifications

  • Master’s or Doctorate in Occupational Therapy from an accredited program.
  • Active Occupational Therapist license in CA.
  • Strong assessment, treatment planning, and patient care skills.
  • Excellent communication, counseling, and interpersonal skills.
  • Ability and willingness to travel to patient locations, if required.
  • Valid driver’s license.

Desired Qualifications

  • Experience providing OT services in home health, outpatient, or community-based settings.
  • Ability to work collaboratively within a multidisciplinary care team.
  • Passion for helping patients achieve independence and improve quality of life.
  • Familiarity with electronic health records and telehealth platforms.
